By completing this form you are requesting an appointment for one of our volunteers to visit the address provided by yourself, in order to conduct a Rehoming Welfare Assessment as part of our Rehoming Assistance Programme.
We will do everything that we can to support your request, but this service is offered subject to the availability of our volunteers. It may be that we need to adjust the preferred timings of the appointments to match availability, and in some circumstances we may need to cancel the appointment if we are unable to identify volunteer availability for your timings or location.
